Job Overview:
The Coordinator for the Indian Medical Program is responsible for building effective working relationships with affiliated and non-affiliated physicians, senior centers, hospital providers, family members/care givers, and community organizations to improve individualized patient care coordination and reduce barriers across the health care continuum.
The Coordinator will coordinate physician office visits, outpatient treatment, surgical and specialist referral appointments, transportation arrangement, accompany patients to offsite physician offices and/or hospital departments to provide language support. The Coordinator is also responsible for community outreach, events, communication, and education.
